29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. a.) Facility - On or about 6/26/13, the floor/walkway of the facility was strewn with cardboard, pieces of wood and equipment pieces. The passageway was not kept in a clean and orderly condition. b.) Facility - on or about 6/26/13, the ledges and horizontal surfaces had accumulations of sawdust from the processing of cardboard and wood products to make animal bedding. Accumulations of dust can be explosive under certain conditions. The employer did not ensure that the place of employment was kept in an orderly and clean condition. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Facility - On or about 6/26/13, an employee was performing repairs on the Rotochopper wood chipping machine (deisel-powered/battery start), without having locked out the energy sources., and the employee has performed repairs and servicing on the Tremzac wood shaving machine. The employer did not establish and implement an energy control program, that includes an energy control procedure, employee training and periodic inspections to ensure that unexpected energization, startup or release of stored energy could occur and cause injuries. ABATEMENT CERTIFICATION REQUIRED
