29 CFR 1926.50(g): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  a. On June, 25, 2013, employees were working with White Portland Cement without an eyewash/quick drench system or clean towels to remove upon contact with the material. The employees were exposed to contact with caustic materials.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment shall not be used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use.   a. On June 25, 2013, employees were working from a raised platform affixed to the front of an all-terrain forklift. The Gradall 534B all-terrain forklift was not designed for the specific platform used.
