The guarding device was not in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, was not so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: This violation was most recently observed on or about June 24, 2013, at the workplace located at 4650 Simonton Road, Dallas, Texas; employees were exposed to amputation hazards while operating a press brake without guards.
