29 CFR 1910.147(c)(7)(iii)(A): Retraining shall be provided for all authorized and affected employees whenever there is a change in their job assignments, a change in machines, equipment or processes that present a new hazard, or when there is a change in the energy control procedures: (a) Employees were not retrained on the energy control program, in that employees thought it was acceptable to lock out the hydraulic presses by attaching locks to the disconnects and then hanging the keys to the locks on hooks close to the disconnects. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(d)(4)(ii): Lockout devices, where used, shall be affixed in a manner that will hold the energy isolating devices in a safe or off position is prohibited: (a) The lockout devices (locks) attached to the disconnects on the hydraulic presses were not attached in a manner that could be relied on to hold the electrical disconnects in the off position, in that the keys to the lockout devices were left hanging on a hook by the disconnects. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
