(b) Sun Well Service Rig #10: On or about June 20, 2013 and at times prior, employees were exposed to hazards such as, but not limited to struck by overhead loads as a result of sling failure due to the unknown sling capacity of the two-legged chain sling used to hoist the Blow Out Preventer.
