29 CFR 1926.451(d)(1): All suspension scaffold support devices, such as outrigger beams, cornice hooks, parapet clamps, and similar devices, shall rest on surfaces capable of supporting at least 4 times the load imposed on them by the scaffold operating at the rated load of the hoist (or at least 1.5 times the load imposed on them by the scaffold at the stall capacity of the hoist, whichever is greater). 1. On Thursday, June 20, 2013, a suspended scaffold holding four (4) men collapsed while the men were performing caulking and window washing duties at the SSM St Mary's medical building in Richmond Heights, Missouri. One employee hanging from a rope had to be rescued from a grate (8 feet tall) of the building by the fire department.

29 CFR 1926.451(d)(2): Suspension scaffold outrigger beams, when used, shall be made of structural metal or equivalent strength material, and shall be restrained to prevent movement. 1. On Thursday, June 20, 2013, the suspension scaffold outrigger beams moved before the suspended scaffold holding four (4) men collapsed. These men were executing caulking and window washing duties from the platform of the suspended scaffold located at the SSM St Mary's medical building in Richmond Heights, Missouri. One employee dangling from a rope was found on top of a grate connected to the building was rescued by the fire department.

On Thursday, June 20, 2013, the outrigger beams of a suspended scaffold were not stabilized by bolts or other direct connections to the roof deck so the suspended scaffold holding four (4) men collapsed while the men were engaged in caulking and window washing activities. One employee dangling by a rope had to be rescued by the fire department from the top of a grate (at least 8 feet tall) connected to the SSM St Mary's medical building in Richmond Heights, Missouri.
