29 CFR 1910.23(d)(1): Flights of stairs having four or more risers were not equipped with a standard railing as specified in paragraphs (d)(1)(i) through (v) of this section: (a) Lucille 3-27H North of Lambert MT: On or about June 20, 2013 and at times prior, employees were exposed to fall hazards while accessing and exiting from the unguarded stairs to the PT 204 trailer.
