On or about 6/24/13, the fourth working platform on the supported tubular welded frame scaffold was missing planks. Employees were performing brick pointing and washing activities from the fourth working platform approximately 26 feet above ground level.

29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length extended over its support more than 12 inches, and was not designed and installed so that the cantilevered portion of the platform was able to support employees and/or materials without tipping, or had guardrails which blocked employee access to the cantilevered end.  a) Jobsite: 1900 Spring Garden St, Philadelphia, PA - Scaffold.  On or about 6/24/13, the cantilevered portion of the platforms on the supported tubular welded frame scaffold extended more than 12 inches over their support which exposed the employees to tipping hazards. Employees were performing brick pointing and washing activities from the outrigger section on the 5th working platform, approximately 32 feet above ground level, and from the 4th working platform, approximately 26 feet above ground level.

On or about 6/24/13, a safe means of access was not provided for employees working from the supported tubular welded frame scaffold. Employees were performing brick pointing and washing activities from the outrigger of the 5th working platform, approximately 32 feet above ground level, and from the 4th working platform, approximately 26 feet above ground level, and accessed the scaffold via the rungs on the integral prefabricated access frames.
