OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish to each of his employees employment and a place of employment which are free from recognized hazards that are causing or likely to cause death or serious physical harm to his employees in that employees are exposed to potential struck-by and crushed-by hazards due to employers non-adherence to Rite-Hite RHR Dok-Lok Operater's Manual requirements to train and instruct employees on safe use of equipment, operation and to follow recommended inspection procedures: This violation most recently occurred on June 12, 2013, in packout areas; where employees operated the Dok-Lok systems and the operators were not trained and instructed on safe use of equipment, operation and the manufacturers recommended inspection procedures. Employees were exposed to potential struck-by and caught-in-between hazards.
