29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): a. East side of building - employees working from a tubular welded scaffold were exposed to an approximate 13 ft fall hazard, in that, the work platforms were not fully decked. Employees were working on four sections of a tubular welded scaffold. Three sections contained 5 planks and one section contained 4 planks. Violation observed on or about 06/26/13.

Southeast corner of building - employees working from a tubular welded scaffold were exposed to an approximate 13 ft fall hazard, in that, employees walked across from the east side section of scaffold to the south side section of scaffold. The space between the planks was 32" wide. Violation observed on or about 06/26/13.
