Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system a) On west side of building: On or about June 27, 2013 and at times prior the employer exposed his employees to fall hazards in that he allowed his employees to work on a 1/2:12 pitch roof without fall protection or safety monitor exposing his employees to falls of 21 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At Northwest corner (rear) of building: On or about June 27, 2013 and at times prior the employer exposed his employees to fall hazards in that employees were allowed to access a roof that was 21 feet above the ground with a ladder that was set up where the ladders side rails did not extend at least 3 feet above the upper landing surface.
