29 CFR 1926.405(b)(1): Conductors entering boxes, cabinets, or fittings. Conductors entering boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed. Unused openings in cabinets, boxes, and fittings shall also be effectively closed.  a). Outdoors, adjacent to Building #1/Clubhouse- On or about 25 June 2013, there was one unused opening inside a 480-volt temporary power outlet box, thus exposing employees to electrical and life safety hazards.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables shall not be used.  a). Building #1/Clubhouse- On or about 25 June 2013, the outer and inner sheathing was damaged on a yellow extension cord connected into a 120-volt receptacle used to energize a portable Porter-Cable Quick-Change Saw (Model 737), thus exposing employees to an electrical shock hazard.
