OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer does not furnish to each of his employees employment and a place of employment which are free from the recognized hazards of being struck-by bolts lifted by two Budgit 1/2 Ton underhung hoist with severe signs of corrosion on the chains and hooks which are causing or likely to cause death or serious physical harm to employees.       This violation was observed on or about June 24, 2013, in the Electro Polish Area where an employee was exposed to being struck-by loads of bolts being lifted by two Budgit 1/2 Ton underhung hoists with severe signs of corrosion on the chains and hooks.        Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps it is taking to ensure that hoist chains and hooks are maintained without corrosion.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedure did not clearly and specifically specify procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:     The employer does not ensure that procedures clearly and specifically specify procedural steps for the control of potentially hazardous energy when employees are engaged in activities covered by this section.     a)   This violation occurred on or about June 24, 2013 in the production area, where press operators and maintenance employees were exposed to struck-by and caught-in hazards when performing set-up or maintenance tasks on the following equipment such as, but not limited to      1.   Bliss S1-200-30-36    2.   Bliss S1-200-30-26   3.   Bliss S1-200-30-46                            b)   This violation occurred on or about June 24, 2013 and at times prior thereto, in the production area where press operators and maintenance employees were exposed to struck-by and caught-in hazards when performing set-up or maintenance tasks on the following equipment such as, but not limited to:     1.  Budgit, 1/2 Ton Hoist, Electro Polish, Pneumatic   2.  Budgit, 1/2 Ton Hoist, Electro Polish, Pneumatic   3.  Dynopower Rectifier, 77126-001-001     4.  Filtration System ( 220147)         Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps and procedures that will be developed, which specify procedural steps for shutting down, isolating, blocking and securing machines to control hazardous energy.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    The employer does not guard the rotating parts of machines whose operation exposes employees to injury.      a). This violation was observed on or about June 24, 2013 in Building 1, threader area,  where employee was exposed to struck-by and caught-in hazards when operating a Landis threader 2.5" , 2L71 without a chuck guard.     b) This violation was observed on or about June 24, 2013 in Building 1, machine shop area, where employee was exposed to struck-by and caught-in hazards when operating a Jones and Lamson #3 vertical lathe, serial number 202307 with a raised chuck guard.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all machines that have exposed rotating parts are guarded.

29 CFR 1910.217(b)(7)(v)(a): Each two hand control for single stroke operations on mechanical power press(es) using part revolution clutches was not arranged by design, construction and/or separation so that the concurrent use of both hands was required.       The employer does not ensure that each two-handed control for single stroke operations on mechanical power presses using part revolution clutches is arranged by design or construction so that the concurrent use of both hands is required.  This violation occurred on or about June 24, 2013 in the fabrication department, where employees were exposed to amputation hazards when using the Bliss forging press without a two-handed control to initiate the stroke.             Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each two-handed control for single stroke operations on mechanical power presses using part revolution clutches is arranged by design or construction so that the concurrent use of both hands is required.
