29 CFR 1926.451(b)(4): Each end of a platform, which was not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches: Market Basket Job Site - On or about 6/24/13, the scaffold platform being used by an employee did not extend over the centerline of the outrigger at least 6 inches as it fell off the support and onto the ground resulting in the employee falling approximately 18'.
