29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, or flying chips and sparks: a) In the facility's machine Shop, located at 5634 Hogue in Houston, Texas, on or about June 27, 2013 - Employees working on the TL- 2 HAAS lathe(Model TL Serial Number 3074567),were exposed to struck-by hazards while working with a machine not equipped with guards. b) In the facility's machine shop, located at 5634 Hogue in Houston, Texas, on or about June 27, 2013 - Employees working on the TL-3W HAAS lathe (Model TL-3W Serial Number 73070),were exposed to struck-by hazards while working with a machine not equipped with guards. c) In the facility's shipping and receiving department, located at 5645 Northdale in Houston, Texas, on or about June 27, 2013 - Employees were exposed to struck-by hazards while working on a Table and Panel Saw (no ID) whose blade was unguarded.
