Capacity,operation, and maintenance instruction plates, tags, or decals were not changed accordingly: Establishment - A fork mounted drum grab was used on the Yale fork truck without the fork truck manufacturer's prior written approval. In addition, a lifting capacity label referencing the fork mounted drum grab attachment was not provided on the fork truck.
