29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  (a) Kitchen - On or about June 27, 2013, the employer failed to complete a hazard assessment to determine the proper personal protective equipment  for employee use.  Employees worked with chemicals such as, but not limited to bleach, which contained sodium hypochlorite, a corrosive chemical with a pH of 12 and requires the use of eye protection. Eye protection was not available.

Employees worked with chemicals including, but not limited to bleach, which contained sodium hypochlorite and had a pH of 12. Eye protection was not available.
