29 CFR 1910.147(a)(2)(ii)(B): An employee is required to place any part of his or her body into an area on a machine or piece of equipment where work is actually performed upon the material being processed (point of operation) or where an associated danger zone exists during a machine operating cycle. a) On or about June 7, 2013, during setup operations on the DOOSAN PUMA 2100Y, the employer did not ensure that employees were protected from hazardous energy, at the point of operation, by controlling the hazardous energy through lockout tagout. b) On or about June 27, 2013, during adjusting or changing of the spindle liner on the DOOSAN PUMA 2100Y lathe, the employer did not ensure that employees were protected from hazardous energy, by controlling the hazardous energy through lockout tagout.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. For approximately six weeks prior and as of June 27, 2013, during production operations on the DOOSAN Puma 2100SY, the employer did not ensure that employees were protected from rotating parts, at the spindle liner, by one or more means of guarding.
