29 CFR 1910.146(c)(9)(i): Each contractor who was retained to perform permit space entry operations did not obtain any available information regarding permit space hazards and entry operations from the host employer:    The employer does not obtain any available information regarding permit space hazards and entry operations from the host employer.  This violation was occurred on or about June 14, 2013, and at times prior thereto, in the Methanol Convertor Reactor 7001 where employees were exposed to atmospheric hazards without the employer having obtained any available information regarding permit space hazards from the host employer.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all information regarding permit space hazards and entry operations are obtained from the host employer.

29 CFR 1910.146(d)(2): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not identify and evaluate the hazards of permit spaces before employees entered:    The employer does not identify and evaluate the hazards of permit spaces before employees enter them.  This violation occurred on or about June 14, 2013, and at times prior thereto, in the Methanol Convertor Reactor 7001 where employees were exposed to an atmospheric hazard when working in a permit space without the employer identifying and evaluating all hazards of the permit space.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all hazards of permit spaces are identified and evaluated prior to employees making entry.

29 CFR 1910.146(d)(3)(iv): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit entry operations.    The employer does not implement the means, procedures, and practices necessary for safe permit entry operations to eliminate or control atmospheric hazards.  This violation was occurred on or about June 14, 2013, and at times prior thereto, in the Methanol Convertor Reactor 7001 where employees were exposed to an atmospheric hazard while working in the reactor without developing and implementing the means, procedures, and practices necessary for safe permit entry operations.       Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that permit spaces are purged, inerted, flushed, or ventilated as necessary to eliminate or control atmospheric hazards.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    The employer does not provide employees effective information and training on hazardous chemicals in their work areas.  This violation occurred on or about June 14, 2013, and at times prior thereto, in the Methanol Convertor Reactor 7001 where employees were exposed to an atmospheric hazard when making entry and working in the reactor, a permit required confined space, without having been provided effective information and training on the health hazards in their work area.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are provided effective information and training on hazardous chemicals in their work area.
