G.E. Unit Batch Area, right side - On or about 6/27/2013, employees utilized a supplied breathing air system during spraying operations. The last change date for the canister filter connected to the breathing air supply line could not be verified as no tag was available.  b.) G.E. Unit Batch Area, left side - On or about 6/27/2013, company employees utilized a supplied breathing air system during spraying operations. The last change date for the canister fllter connected to the breathing air supply line could not be verified as no tag was available.

29 CFR 1910.134(i)(7): Oil lubricated compressor(s) used to supply breathing air did not have a high-temperature or carbon monoxide alarm(s) or both:    a.) Air Compressor, Compressor Area Rear of Building - On or about 6/27/2013, employees utilized a supplied breathing air system during spray coating operations in the G.E. Unit Batch Area. The carbon monoxide detector for the system had been removed for calibration.  A replacement detector had not been installed.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.   a.) Eye Wash between coating lines - employees were exposed to a sulfuric acid etch mist near the beginning of the first coating line.  The eyewash unit in the area was not maintained properly in that there was a low flow of water from the eye wash fixtures to the extent that the water would not have reached an injured individual's face.
