29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:   On or about June 28, 2013, at the job site located along Springhill Rd in Benton, Arkansas, the employer did  not provide employees who were working in a trench in excess of 4 feet deep, a safe means of egress that would require no more than 25 feet of lateral travel exposing employees to caught in and crushing hazards in the event of a cave in.

On or about June 28, 2013, at the job site located along Springhill Rd in Benton, Arkansas, the employer did not ensure the spoil pile was staged at least 2 feet from the edge of the excavation, exposing employees to struck by and crushing hazards.
