Facilities Dock; on or about 26 June 2013 and times prior thereto, the employer did not ensure that a North American, Post Mounted Crane, Model MCT, Serial # MCT-220-96-1210 had a load rating chart. This condition exposed employees to a struck by hazard.

Facilities Dock; on or about 26 June 2013 and times prior thereto, the employer did not ensure that the maximum safe load rating for the facilities dock was posted. This condition exposed employees to a struck by hazard.
