29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): (a) Jobsite, 912 Portland Street, Pittsburgh, PA: On or about 06/28/13 the working levels on the scaffold, approximately 15 feet above the ground were not fully planked between the front uprights. Employees were working on the platforms that consisted of 2 planks exposing them to potential fall hazards.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: (a) Jobsite, 912 Portland Street, Pittsburgh, PA: On or about 06/28/13 fall protection was not in place to protect employees working on the scaffold. There were no guardrail systems or personal fall arrest systems in place to prevent the employees from falling approximately 13-15 feet to the ground.

29 CFR 1926.454(c)(3): The employer failed to retrain employees that showed indications through work performance of not retaining the requisite proficiency: (a) Jobsite, 912 Portland Street, Pittsburgh, PA: On or about 06/28/13 the employer did not retrain employees when deficiencies in work performance indicated the employees had not retained the requisite proficiency of working safely on a scaffold. Employees were working on a scaffold with the following deficiencies: no fall protection and inadequate platform and decking. The employees had not been retrained to ensure they understood the necessary requirements to work from a scaffold in a safe manner.
