29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.  a) North Side of the Building -A roofer was observed without head protection while on a roof applying a sealant exposing him to a 35 foot fall hazard, on or about 06/26/13. 29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) North Side of the Building - A roofer was observed without fall protection while on a roof applying a sealant exposing him to a 35 foot fall hazard, on or about 06/26/13.  b) West Side of the Building - A roofer was observed without being tied off while on a roof applying new tile cap exposinghim to a 35 foot fall hazard, on or about 06/26/13. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed  a) South Side of the Building - A foreman and a roofer were observed using an extension ladder which only extended 30 inchespast the edge of the roof exposing them to a potential 30 foot fall hazard, on or about 06/26/13.   No Abatement Certification Required
