Condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.501(b)(13): Each employees engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) San Buena Construction at 8604 Martin Luther King Blvd., Denver, CO 80238: On or before June 29, 2013, the employer did not ensure that employees were protected from fall hazards in the following instances: 1) An employee accessed a second floor porch landing that was missing guardrails; and 2) Employees were working on the main floor that was missing guardrails around the stairway landing over the basement. Conditions exposed employees to fall hazard of up to 12 feet 9 inches. San Buena Construction was previously cited for a violation of this occupational safety and health standard or its equivalent, 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 311905707, citation number 1, item 3, issued September 30, 2008 and became a final order on November 2, 2008, with respect to a workplace located at 3131 Zuni Street, Denver, CO 80211.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) San Buena Construction at 8604 Martin Luther King Blvd., Denver, CO 80238: On or before June 29, 2013, the employer allowed employees to use two yellow extension cords that were both missing ground plugs, used to power two 120 volt circular saws. The employer also allowed the employees to use a 120 volt Hitachi air compressor, model EC 12, that was missing its ground plug. Condition exposed employees to electrical shock hazards.
