Kasp Inc. dba Servpro of West Erie County, 227 College Street, Youngsville, PA, 16371-Gymnasium area-on or about June 28th, 2013, and times prior thereto: Company personnel used disposable dust masks (3M brand 8000 series) voluntarily during cleanup operations in the Youngsville High School gymnasium. The employee's that used the dust masks were not provided with the information in Appendix D to this section.

Kasp Inc. dba Servpro of West Erie County, 227 College Street, Youngsville, PA, 16371-Gymnasium area-on or about June 28th, 2013, and times prior thereto: Company personnel were utilizing tight fitting, half mask respirators during cleaning operations onsite. The company did not maintain medical records regarding the employee's ability to wear a respirator.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a.) Kasp, Inc. dba ServPro of West Erie County,-Wrestling room of Youngsville High School-on or about June 28th, 2013, and times prior thereto: Company personnel were working with and around hazardous substances including but not limited to, SERVPRO Green, and the contents of discharged ABC Dry chemical fire extinguishers. Hazard Communication training had not been conducted with all company personnel at the worksite.
