29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered.    a) Warehouse - On or about 6/28/13, no protective equipment was provided to protect the employees from fall hazards capable of causing injury and impairment.  Employees performed tire weaving operations on stacks of weaved tires approximately 7 ft. above hard surface floors.

29 CFR 1910.305(g)(1)(iv): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used: (A) As a substitute for the fixed wiring of a structure; (B) Where run through holes in walls, ceilings, or floors; (C) Where run through doorways, windows, or similar openings; (D) Where attached to building surfaces; (E) Where concealed behind building walls, ceilings, or floors; or (F) Where installed in raceways, except as otherwise permitted in this subpart      a) Warehouse - On or about 6/28/13, flexible cords were used as a substitute for the fixed wiring of the pipe warmer in the lower building of the warehouse, and of the lighting fixtures in the warehouse exposing employees to electrical and fire hazards.    b) Warehouse - On or about 6/28/13, flexible cords were run through the doorway in the lower building exposing employees to electrical and fire hazards.    c) Warehouse - On or about 6/28/13, flexible cords were attached to structural members of the ceiling and the mezzanine in the warehouse, and the vertical beam by the front desk in the warehouse, exposing employees to electrical and fire hazards.
