29 CFR 1926.452(j)(3):  A workbench was used as a top guardrail even though it did not meet all of the guardrail requirements specified in 1926.451(g):    a)  B Diaz Construction Inc. at 2703 Kenton Street, Denver CO 80238:  On or about June 29, 2013, a work bench used as a top guardrail was 31 inches above the working platform.  This work bench was not installed between 38 inches and 45 inches high as required in 1926.451(g).  Condition exposed employees to fall hazards of 15.6 feet.    Abatement note:  Standard guardrail requirements include a top rail set between 38 and 45 inches high, and a mid-rail set half way between the working platform and the top rail.  End rails are also required.

This extension cord was used to power a 120 volt Dura Air air compressor, and a 120 volt Ridgid circular saw, model Mg, serial number BB0547 65828.  These tools were on a ground fault circuit interrupter (GFCI).  Condition exposed employees to electrical shock hazards.
