29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.    (a)  On June 27, 2013, at a job site located at 4220 Main St, Kansas City, MO., employees were exposed to serious injuries, in that the scaffold platform used to access the upper work area was approx. 3 feet below the point of access.    Abatement certification for this violation is not required.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a)  On June 27, 2013, at a job site located at 4220 Main St, Kansas City, MO., employees were exposed to serious injuries, in that the mobile scaffold in use on the north side of the work area was not enclosed on all open sides of the work platform. As a result, employees were exposed to approx. 10 to 11 ft. fall to the concrete surface below.    Abatement certification for this violation is not required.

29 CFR 1926.454(a)(3): The employer did not ensure that each employee who performed work while on a scaffold was trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the proper use of the scaffold, and the proper handling of materials on the scaffold:  2(a)  On June 27, 2013, at a job site located at 4220 Main St, Kansas City, MO., employees were exposed to serious injuries, in that they were not trained to recognize the fall and access hazards associated with mobile scaffold in use on the north side of the work area. As a result, employees were exposed to approx. 10 to 11 ft. fall to the concrete surface below.    Abatement certification for this violation is not required.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a)  On June 27, 2013, at a job site located at 4220 Main St, Kansas City, MO., employees were exposed to serious injuries, in that the unprotected sides and edges of the elevated work area was not provided with guardrails or any other means of fall protection. The employees were exposed to an approx. 13 to 14 ft fall hazard to the concrete surfaces below.    Abatement certification for this violation is not required.
