29 CFR 1910.1030(f)(1)(i): The employer did not make available the hepatitis B vaccine and vaccination series to all employees who have occupational exposure:    a) Harbor Healthcare and Rehabilitation Center, 301 Oceanview Boulevard, Lewes, DE -  The Hepatitis B vaccination was not made available to all nurses and certified nursing assistants with potential exposure to blood borne pathogens while performing nursing activities such as, but not limited, handling blood contaminated materials.  On or about  July 10, 2013.

29 CFR 1910.1030(g)(2)(ii): The employer did not ensure that training was provided to employees with occupational exposure at the time of initial assignment to tasks where occupational exposure might take place and at least annually thereafter:  a) Harbor Healthcare and Rehabilitation Center, 301 Oceanview Boulevard, Lewes, DE - Employees such as, but not limited to nursing employees, with potential exposure to blood borne pathogens, were not trained in accordance with this requirement.  On or about July 10, 2013.  1.  Initial training for seventeen (17) employees was not provided.    2. Training for five (5) employees was not provided within a year after their initial training.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:    Harbor Healthcare and Rehabilitation Center, 301 Oceanview Boulevard, Lewes, DE (Electrical Utility Room) - The area in front of the electrical panel and switches was used for storage, preventing sufficient access to this equipment.  On or about July 2, 2013.    Harbor Healthcare was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(g)(1), which was contained in OSHA inspection number 311808356, citation number 1, item number 1, and was affirmed as a final order on May 25, 2010, with respect to a workplace located at 301 Oceanview Boulevard, Lewes, DE 19958.

29 CFR 1910.303(b)(2):  Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling:  a)  Harbor Healthcare and Rehabilitation Center, 301 Oceanview Boulevard, Lewes, DE (Trash Dumpster Area) - Energized Romex 14/2 electrical wire running across the ground to a lamp junction box mounted on a wooden pole adjacent to a trash dumpster, was subject to physical damage, exposing employees to an electrical hazard.  On or about July 2, 2013.
