29 CFR 1910.22(d)(2):  A load greater than that approved by the building official was permitted on the roof of a building or other structure:        a) Feed Bins:  On or about June 24, 2013, and at times prior thereto, the roof of the feed bins was not maintained and had rusted.  Saw dust was allowed to accumulate in large volumes on the failing roof, thereby exposing employees to potential struck-by hazard.         Note: Abatement certification is required for this item.

29 CFR 1910.23(e)(1): A standard railing did not consist of top rail, intermediate rail, and posts, and have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level:     a) On the exterior heat exchanger cat walk on the boiler building: On June 24, 2013, and at times prior thereto, the cat walk was missing the mid rail and one end rail between the new and the added walkway.        b) In the boiler building under the flyash venturi: On June 24, 2013, and at times prior thereto, the work platform (16 inch wide concrete ledge)  where the fly ash venturi was cleaned did not have a complete guard rail to prevent employees from falling 50 inches to the concrete floor. 29 CFR 1910.269(e)(4): Before any entrance cover to an enclosed space is removed, the employer did not determine whether it was safe to do so by checking for the presence of any atmospheric pressure or temperature differences and by evaluating whether there might be a hazardous atmosphere in the space. Any conditions making it unsafe to remove the cover were not eliminated before the cover is removed.       a) At Pit 3 north of the boiler building: On June 24, 2013, and at times prior thereto, employees were instructed to check the pump oilers inside the Pit 3 building twice daily without checking for hazards such as steam, fog, or lack of oxygen. 29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    a) On the bench grinder in the Boiler building: On June 24, 2013 and at times prior thereto, the tongue guard on the Milwaukee bench grinder in the maintenance area  was missing.    Note: Abatement certification and documentation ARE required for this item.   Tamarack Mills, LLC dBA Evergreen Forest was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.215(b)(9), which was contained in OSHA inspection number 311578769 citation number 01, item number 001 and was affirmed as a final order on January 22, 2009, with respect to a workplace located at 3555 Hwy 95, New Meadows, ID 83654.
