29 CFR 1910.213(a)(9): All belts, pulleys, gears, shafts, and moving parts shall be guarded in accordance with the specific requirements of 1910.219. Maintenance shop; on or about 26 June 2013 and times prior thereto, an Oliver Wood Planer, Machine number 299-D, Serial Number 58493, was not guarded. The guard covering the belt drive was removed. This condition exposed employees to a caught in hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws. Maintenance shop; on or about 26 June 2013 and times prior thereto, two instances where strain relief was not provided: Instance (a) - an Air Master Circular Fan, Model K55HXJHE-2870 was not connected to a device and fitting so that strain relief was provided. This condition exposed employees to an electrical shock hazard. Instance (b) - Maintenance shop; on or about 26 June 2013 and times prior thereto, an Sears, Craftsman, 7 inch Metal Cutting Band Saw, Model 113.229670, Serial number 0161.P0009 was not connected to a device and fitting so that strain relief was provided. This condition exposed employees to an electrical shock hazard.

29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit". Warehouse ramp loading area, Paint shop; on or about 26 June 2013 and times prior thereto, two instances were noted where exit signs were not posted along the exit route. This condition exposed employees to smoke inhalation and fire hazards: Instance (a) Warehouse exit door next to overhead roll-up door along exit route was not marked. Instance (b) Paint shop exit door along exit route was not marked.
