On or about June 25, 2013, on the north side of the building employees landing material from the thrid floor balcony were not protected from a 33-foot fall hazard by the use of a fall protection system. b. On or about June 25, 2013, on the north side of the building employees landing material from the fourth floor balcony were not protected from a 44-foot fall hazard by the use of a fall protection system.
