OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards from falling parts due to the lifting devices not being marked as to their capacities.    a. Reliable Castings Corporation located in Sidney, Ohio:  On or about June 26, 2013, the employer did not ensure the hook being used to lift castings from the shakeout table in the sand foundry was marked as to its rated capacity.    b. Reliable Castings Corporation located in Sidney, Ohio:  On or about June 26, 2013, the employer did not ensure the spreader bar and basket used to transfer parts on the impregnation line located in the machine shop was marked as to its rated capacity.    Among other methods, one feasible and acceptable method to correct this hazard is to ensure the under the hook lifting device is labeled according to ANSI B30.20.

Reliable Castings Corporation located in Sidney, Ohio:  On or about June 26, 2013, the employer did not ensure safety data sheets were readily available to employees who work with substances such as but not limited to combustible aluminum dust and respirable crystalline silica.  Safety data sheets were stored on computers located in managers offices which required managers to be present for employees to access the information.

The steel alloy chain sling had a severely damaged master link and the four legs of the chain sling were each equipped with quick links.     b.  Reliable Castings Corporation located in Sidney, Ohio:  On or about June 26, 2013 the employer did not ensure the steel alloy chain sling used to move 350 pound molds from the Taber molding line to the roller conveyor in the sand foundry was inspected daily by a competent person.  The master link for the four foot steel alloy chain sling was severely stretched and elongated.
