29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person: a) Construction Site; On or about July 1, 2013 and at times prior, employees standing on a wooden pallet being suspended on the forks of an all-terrain forklift, were exposed to a fall of approximately six feet to the ground below with a subsequent hazard of being struck by the pallet and saw while cutting and tying re-enforcement bar.

29 CFR 1926.451(f)(5): Scaffolds were moved horizontally while employees were on them although the scaffolds were not designed for such movement by a registered professional engineer: a) Construction Site; On or about July 1, 2013 and at times prior, employees standing on a wooden pallet being suspended on the forks of an all-terrain forklift, were exposed to a fall of approximately six feet to the ground below with a subsequent hazard of being struck by the pallet and saw while moving into position for the cutting and tying of re-enforcement bar.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a) Construction Site; On or about July 1, 2013 and at times prior, employees were exposed to a fall of approximately six feet to the ground below with a subsequent hazard of being struck by the pallet due to lack of adequate training in the recognition of fall hazards and the means to control these hazards.
