29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a. On or about July 1, 2013, the employer had not established a machine specific lockout/tag-out procedure that had been written and implemented for the performance of maintenance or repair on the Feeding Ironers Machine where the unexpected energizing, startup or release of energy sources could occur and cause injury.

On or about July 1, 2013, at the Dirty Laundry Washing Area, an employee was using the corrosive chemical, bleach, the employer did not provide an eye wash station for the quick drenching or flushing of the eyes within the work area, for immediate emergency use.      b.  On or about July 1, 2013, at the Dirty Laundry Washing Area an employee replacing a 50-gallon container of corrosive chemical, AdvaCare Sanitizer, did not have an eye wash station for the quick drenching or flushing of the eyes within the work area, for immediate emergency use.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:     a.         On or about July 1, 2013, above the table, next to a small folding machine in the shop, an extension cord had been plugged into an electrical outlet on the floor and run up above the table was being used as a substitute for permanent wiring to energize the small folding machine, a fan and a small hand tool.        b.         On or about July 1, 2013, next to the table, close to the North East side of Exit door in the shop, an extension cord had been plugged into an electrical outlet above the machine, and ran down to the floor, where it was used as a substitute for permanent wiring to energize a small press machine.       c.         On or about July 1, 2013, on the Blanket Machine, an extension cord, which had been plugged into an electrical outlet above and connected to a fan, was used as a substitute for permanent wiring to energize the fan.
