29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) Front lower roof, new residential construction located at 1313 Blairwood Court, Atlanta, GA 30331: Employee was exposed to falls of approximately 12-feet, 4-inches while installing vinyl soffit from a 4/12 pitched roof with no fall protection system in place.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: (a) Front lower roof, new residential construction located at 1313 Blairwood Court, Atlanta, GA 30331: Employee was exposed to falls of approximately 12-feet, 4-inches while installing vinyl soffit from a closed Werner 10-foot A-Frame/Step Ladder on a 4/12 pitched roof which was resting on a 2x4 cleat. The ladder was not used as intended by the manufacturer as it was used on a surface which was not level and not used in the open position supported by all four feet.
