29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):  (a) Mill: The Toyota Propane Forklift #12 did not have a nonslip surface for the brake pedal and the forklift was not taken out of service until it was repaired.  (b) Mill: The Daewoo Forklift #4 did not have a nonslip surface for the brake pedal, and the horn did not work and the forklift was not taken out of service until it was repaired.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a) Employees were exposed to hazardous chemicals and there was no written hazard communication program to communicate how chemicals were to be labelled, material data sheets were to be obtained, and employee training would be conducted.  Employees were exposed to hazardous chemicals, such as, but not limited to, Crown MEK (Methyl Ethyl Ketone, and CEMGUARD Plus White Pre-Catalyzed Enamel Base which are both extremely flammable.
