29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  (a) Second Floor, new residential construction located at 2635 Reece Farms Trail, Powder Springs, GA 30127: Employees nailing down a plywood sub-floor were exposed to eye injuries while operating a Hitachi (Model NR83A2-S1) 3 1/4-inch Strip Nailer and a Bostitch (Model RN46-1) Coil Nailer without safety goggles or glasses.

29 CFR 1926.300(b)(2): Moving part(s) of equipment exposed to contact by employees or which otherwise created a hazard, were not guarded:  (a) Front side of new residential construction located at 2635 Reece Farms Trail, Powder Springs, GA 30127: Employees using a Jenny (Model K5HGA8) 8-Gallon Wheelbarrow Air Compressor were exposed to amputation hazards when the moving belts and pulleys were not guarded as the belt guard was not in place during operation.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Second floor, new residential construction located at 2635 Reece Farms Trail, Powder Springs, GA 30127: Employees engaged in residential framing activities were exposed to falls of approximately 12-feet, 2-inches when a fall protection system was not provided on the second floor working surface.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) Front side, new residential construction located at 2635 Reece Farms Trail, Powder Springs, GA 30127: The ladder provided was not secured to the structure and the side rails of the ladder did not extend at least 3 feet above the working surface to allow for safe access on and off the second floor. Employees accessing the ladder were exposed to falls of approximately 12-feet, 2-inchs to the lower level.
