They did not have not more than 1/4 inch (0.6 cm) clearance above the walking/working surface. They did not have solid or have openings not over 1 inch (2.5 cm) in greatest dimension: a. On or about 7/2/13, North Building Side, at the 2nd story clubhouse floor, toeboards installed at the floor edge, did not provide at least 3.5 inches of vertical height from their top edge to the level of the walking/working surface and employees working at ground level were exposed to struck-by hazards.
