29 CFR 1910.23(e)(1): A standard railing shall consist of top rail, intermediate rail, and posts, and shall have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level. The top rail shall be smooth-surfaced throughout the length of the railing. The intermediate rail shall be approximately halfway between the top rail and the floor, platform, runway, or ramp. The ends of the rails shall not overhang the terminal posts except where such overhang does not constitute a projection hazard.  a. On or about July 03, 2013, at the above located workplace, employees in the upstairs storage area were required to access and work on an open sided platform approximately seven (7) feet high. The platform did not contain a standard railing including, but limited to a vertical height of 42 inches and an intermediate rail. Employees were thereby exposed to the hazards associated with falls.

29 CFR 1910.23(e)(2): A stair railing shall be of construction similar to a standard railing but the vertical height shall be not more than 34 inches nor less than 30 inches from upper surface of top rail to surface of tread in line with face of riser at forward edge of tread.  a. On or about July 03, 2013, at the above located workplace, employees were required to access the upstairs storage room.  On the open side of the stairway a handrail was not in line with the face of riser at forward edge of tread. Employees were exposed to the hazards associated with falls.

On or about July 03, 2013, at the above located workplace, employees were required to operate a powered industrial vehicle (Caterpillar EC25K) during material handling operations. The employer did not ensure that each powered industrial truck operator completed the required training and evaluation specified in paragraph (l).

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  a. On or about July 03, 2013, in the lunchroom of the facility, an unused opening in the electrical junction box to operate the lights was not effectively covered. Employees in the facility were thereby exposed to the hazards associated with electrical contact.

29 CFR 1910.305(g)(1)(iv)(A): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cable may not be used:  A) As a substitute for the fixed wiring of a structure;  a. On or about July 03, 2013, in the shop area of the facility, flexible cords were used as a substitute for fixed wiring to provide electricity for a permanent lamp. Employees were exposed to the hazards associated with electrical fires.

On or about July 03, 2013, at the above located workplace, employees were required to assemble parts inside the shop area. The work area was not kept orderly in that semi-truck assembly parts and other items accumulated on the surrounding floor. Employees were exposed to the hazards associated with trip and falls.

On or about July 03, 2013, at the above located workplace, employees required to clean parts were exposed to the hazards associated with chemicals such as, but not limited to, industrial solvent containing petroleum. These hazards necessitated the use of personal protective equipment such as safety glasses and gloves to prevent occupational injuries or illnesses. The employer had not verified, through written certification, that a hazard assessment had been performed.
