29 CFR 1910.95(c)(1): 29 CFR 1910.95(c)(1):  A continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (n) was not instituted when employee noise exposures equaled or exceeded an 8-hour time-weighted average (TWA) sound level of 85 dBA:      An effective hearing conservation program which included noise monitoring, annual audiomettric testing, posting of the Noise Standard 29 CFR 1910.95, and annual training was not instituted by Duro Cast, Inc.     a) On or about July 15, 2013, a furance tender was exposed to an 8-hour TWA noise level of 89.11 dBA with a dose of 88.4% during a 412 minute sampling period allowing 68 minutes for zero exposure.    b) On or about July 15, 2013, a furance tender/pourer was exposed to an 8-hour TWA noise level of 88.60 dBA with a dose of 82.4% during a 303 minute sampling period allowing 177 minutes for zero exposure.    c) On or about July 15, 2013, a furance tender/pourer/shakeout operator was exposed to an 8-hour TWA noise level of 91.37 dBA with a dose of 120.9% during a 434 minute sampling period allowing 46 minutes for zero exposure.    d) On or about July 15, 2013, in the Finishing Department an employee operating the bandsaw followed by the belt sander was exposed to an 8-hour TWA noise level of 90.69 dBA with a dose of 110% during a 407 minute sampling period allowing 73 minutes for zero exposure.    e) On or about July 15, 2013, in the Finishing Department an employee operating the belt sander was exposed to an 8-hour TWA noise level of 87.70 dBA with a dose of 73.0% during a 402 minute sampling period allowing 78 minutes for zero exposure.    f) On or about July 15, 2013,  in the Finishing Department an employee conducting core knockout followed by operating the bandsaw was exposed to an 8-hour TWA noise level of 88.41 dBA with a dose of 80.2% during a 388 minute sampling period allowing 92 minutes for zero exposure.
