29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) In the trench excavation - when installing a storm pipe an employee was exposed to a cave-in hazard in that, a safe means of egress was not provided in the trench which was 8' feet in depth, on or about June 5, 2013.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. (a) At the edge of the trench - an employee was installing a storm pipe where the trench measured 8-ft deep. A spoil pile was staged at the edge of the trench, posed a hazard, in that, the trench was not sloped for class "C" soil, exposing the employee to engulfment and struck-by hazards from soil falling or rolling into the trench, on or about June 5, 2013.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that the: (a) In a trench measuring 50 feet long, 12.6 feet wide and 8 feet deep - During the installation of a storm pipe an employee was required to work in an unprotected excavation next to an active roadway in type "C" soil. A protective system (shoring, shielding or sloping) was not provided for the employee, observed on or about June 5, 2013.
