OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that they were exposed to an evacuation hazard: a) Keystone Human Services, Nassau Road Home - Employees were required to assist non-ambulatory residents to exit the facility in the event of an emergency, and the doorways leading from the patients' bedrooms were not wide enough to assure clearance for wheelchairs, on or about July 3, 2013. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.22(a)(2): The floor of every workroom was not maintained in a clean and, so far as possible, a dry condition. Where wet processes were used, drainage was not maintained, and false floors, platforms, mats, or other dry standing places was not provided where practicable: (a) Nassau Group Home - The floor of the shower did not have a water retainer to prevent water from flowing onto the bathroom floor causing a slip hazard for employees who are required to bathe residents, on or about July 3, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
