29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling:   a) On or about 06/27/13, at Cruz Bay Creek Bulkhead, St John, VI, in the cargo inspection area, an uninterruptible power supply was not connected to a permanently installed branch circuit receptacle.   b) On or about 06/27/13, at Cruz Bay Creek Bulkhead, St John, VI in the cargo inspection area, a relocatable power tap was not directly connected to a permanently installed branch circuit receptacle.  c) On or about 06/27/13, at Cruz Bay Creek Bulkhead, St John, VI in the supervisor's office area, a relocatable power tap was not directly connected to a permanently installed branch circuit receptacle.  d) On or about 06/27/13, at Cruz Bay Creek Bulkhead, St John, VI, in the kitchen area, a ground-fault circuit interrupter was not installed in all 125-volt, single phase, 15-and 20-ampere receptacles installed within 6 feet or 1.8 meters of the outside edge of a sink.   PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear:  a) On or about 06/27/13, at Cruz Bay Creek Bulkhead, St John, VI, in the breakroom area, an electrical outlet box was not covered.  b) On or about 06/27/13, at Cruz Bay Creek Bulkhead, St John, VI, in the primary inspection area, an electrical outlet box was not covered.  c) On or about 06/27/13, at Cruz Bay Creek Bulkhead, St John, VI, in the primary inspection, an electrical junction box was not covered.  d) On or about 06/27/13, at Cruz Bay Creek Bulkhead, St John, VI, in the secondary/agriculture area, an electrical outlet box was not covered.  e) On or about 06/27/13, at Cruz Bay Creek Bulkhead, St John, VI, in the hallway area, an electrical junction box was not covered.
