29 CFR 1926.501(b)(13):   Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On the West side of the roof -- during the application of stucco, fall protection was not required for employees working on a roof 10-ft from the eave to ground. The pitch of the roof was 5/12, on or about June 26, 2013.

29 CFR 1926.451(b)(1):   Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a) On the West side - During the application of stucco employees were working on a scaffold that was not fully planked, exposing employees to a 13-8-ft fall hazard, on or about June 26, 2013.

29 CFR 1926.451(c)(3): Supported scaffold poles, legs, posts, frames, and uprights were not plumb and braced to prevent swaying and displacement.  (a) On the west side of the building - during the application of stucco an employee was working on a platform that was supported by a frame frame scaffold that was leaning against the vertical wall of the building an angle on the roof of the overhang with a pitch of 5/12 with only two of the four legs partially anchored, exposing employees to fall hazards 10-ft, on or about June 26, 2013.

29 CFR 1926.451(e)(1):   When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    a) West side of the building - an employees was exposed to a fall of 13.8 feet while using the end frames to access and egress the scaffold, on or about June 26, 2013.
