OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which are free from recognized hazards that are causing or likely to cause death or serious physical harm to his employees in that employees were exposed to struck-by and fall hazards resulting from the use of an improvised, job-built personnel platform which was attached to a Gradall, Model No. G9-43A lift truck.     This violation most recently occurred on July 03, 2013, at a workplace located at 1505 Knoll Manor, Dallas, TX 75206; where at least one (1) employee worked from an improvised, job-made personnel platform that was being lifted by the forks of a Gradall, Model No. G9-43A lift truck.

29 CFR 1926.404(e)(1)(i): Conductors rated 600 volts, nominal, or less did not have sufficient ampacity to carry the electrical load:        This violation most recently occurred on July 03, 2013, at a workplace located at 1505 Knoll Manor, Dallas, TX 75206; where at least one (1) employee used a 12/3 AWG electrical extension cord that did not have sufficient ampacity to carry the electrical load.  The extension cord showed signs of deformation.
