29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502. On or about June 28, 2013, employees who were installing roofing material were not provided fall protection. The employees were exposed to a fall hazard at a height of over 6 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about June 28, 2013, employees were using an aluminum extension ladder with side rails that did not extend at least 3 feet above the edge of the roof. The employees were exposed to a fall hazard.
