29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed. 1. Inside of the boiler room, on the south wall, there was a 480 volt electrical panel box (MSP 9 J) with an unused opening located inside of the breaker box. Employees were exposed to physical contact with live parts. 2. Inside of the maintenance shop, there was a 120/240 volt electrical panel box with two (2) unused openings located inside of the breaker box.

All new overhead and gantry cranes constructed and installed on or after August 31, 1971, shall meet the design specifications of the American National Standard Safety Code for Overhead and Gantry Cranes, ANSI B30.2.0-1967, which is incorporated by reference as specified in Sec. 1910.6. 1. At the time of the inspection, in the recycling area, employees were operating the recycling machine with no safety tag in place above the pendant control box.

Inside of the boiler room, on the south wall, there was a 240 volt panel box with breakers inside that were not marked to indicate the purpose of each breaker located inside of the box. 3. On the shipping and receiving wall, there was a 480 volt panel box (MSP-1-3S) with breakers that need to be marked to indicate the purpose of each breaker. 4. On the shipping and receiving wall, there was a 240 volt panel box with breakers that need to be marked to indicate the purpose for each breaker situated inside of the box. 5. On the shipping and receiving wall, there was another 240 volt panel box with breakers that need to be marked to indicate the purpose for each breaker housed inside of the box. 6. Behind the line 3 filler area, near the anhydrous ammonia tank, there was a 480 volt panel box with breakers that need to be marked to indicate the purpose of each breaker contained inside of the box.
