29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one half hour: a) Outside south end of shop; On or about June 5, 2013 and at times prior thereto, the employer exposed his employees to fire hazards in that employees were permitted to work in the same area where oxygen cylinders were stored within 5 feet of fuel gas (acetylene and propane) cylinders without a barrier as required by the standard.

29 CFR 1910.1030(f)(1)(i): The employer did not make available the hepatitis B vaccine and vaccination series to all employees who have occupational exposure: a) Throughout the Fabrication Shop; On or about June 5, 2013 and at times prior thereto, the employer exposed his employees to potentially becoming infected with hepatitis b in that employees who had been trained in CPR/1st Aid were expected to render CPR/1st Aid in case of an accident had not been offered the hepatitis B vaccination series as required by the regulation.
